Necessary Paperwork

Student with CPS Email, complete SY 23-24 Payton Athletic Player Record Packet 

Documents Required to be Uploaded in Each Player Record Packet (Section 3):

1. IHSA Pre-Participation Physical 

1. Must turn in page signed and dated by Physician- with approval to participate
   
2. Physical is valid for 395 after examination date
   
3. CVS and other Minute-Clinics offer walk-in appointments for physicals

2. WPCP Student-Athlete Policy Acknowledgement Form SY 23-24

1. Must be signed by Parent/ Guardian and Student-Athlete

3. Media Consent Form & Release

1. This form allows the school to livestream athletics and post pictures of athletes on social media. Please express to your coach if you do not approve this.
